In the Newsletter we're going to go over:
  • Review of the DAOM SAP
  • Fall Mentorship, Preceptorship, and Service Learning forms
  • Introduce Clinic Hours Tracker

Reminder:  Submit Summer Online Mentorship, Preceptorship, Service Learning Forms

This is a reminder to submit your Mentorship, Preceptorship, Service Learning Forms for  the  Summer Quarter.   Reminder that these forms  have now been condensed into 2 separate forms:  APPLICATION and EVALUATION.  

These Forms will allow fellows to fill out their applications for Mentorships, Preceptorships, and Service Learning opportunities.

DAOM SAP and Clinical Hours

As a reminder, in Winter Quarter of 2018, the program revised the Satisfactory Academic Progress (SAP) requirement (found in the Student Handbook https://www.emperors.edu/publications/) to read as follows:
SATISFACTORY ACADEMI C PROGRESS (SAP)
There are three requirements to maintain Satisfactory Academic  Progress (SAP).
  • Completion of scheduled program requirements.
    • Successful completion of the Capstone Project proposal by the end of 1st year (12-month period from start date).
    • Quarterly successful completion of all scheduled Medical Presentations and Clinical Rounds,
    • Quarterly successful completion of a minimum of 50 clinical hours of either Mentorship, Preceptorship, Service Learning).
  • Maintain the minimum required GPA:
    • Minimum quarterly grade point average (GPA) of 3.0.
    • Minimum cumulative GPA of 3.0.
  • Adherence to the attendance policy.
The clinical hour requirement was implement to comply with regulartory requirements and also to keep students on track with their clinical hours for ontime completion.  Each DAOM fellow is requried to complete 650 Clinical Hours, which includes 100 hours Clinical Rounds, and 100 Medical Presenations.  The remaing 450 hours is set aside for DAOM fellows to use to develope their own Mentorship, Preceptorship, and Service Learning Projects.  

Starting Fall Quarter, we'll be enforcing the 50 minimum for students clinical hours, inorder to keep students on track towards graduation, i.e. not trying to complete all of your hours in your last quarter.  Your 50 hour minimum per quarter can be completed through either a Mentorship, Preceptorship, or Service Learning Project by completing the applications below.  

Thank you to everyone who have been submitting your Applications and Evaluations in a timely manner.  If you have an ongoing project that you've started in a previous quarter, that will continue into the Fall, there is no need to fill out and additional Application.  The due date to submit new Fall Applications for your project is Friday 9/28/18.

The Mentorship, Preceptorship, Service Learning Forms for the FALL QUARTER are now available!  Reminder that these forms have now been condensed into 2 separate forms:  APPLICATION and EVALUATION.

NOTE:  FOR OLDER PROJECTS, PLEASE USE THESES FORMS TO COMPLETE THEM.  Detailed instructions can be found in the following sections.
Fall Application Forms

Use the APPLICATION for new projects that you are starting during the FALL QUARTER.  Please remember to work directly with your Supervisors and Mentors directly when filling out the application forms.  

You only need to complete the application once per project.  For example if you're conducting the Didactic Preceptorship in Acupuncture Techniques for Summer and Fall Quarters, you'll only fill out the application once.  However, if you're in Acupuncture Techniques Summer, then Acupuncture Anatomy Fall Quarter, you'll need to fill out 2 applications.  

Once you click submit, your forms are emailed to the DAOM Office for review.  If approved you'll be sent the application back for signatures via DocuSign (https://account.docusign.com/#/username).    

Remember to submit your Supervisor or Mentors CV/Resume.  This may be uploaded to your student folder, i.e. John Smith DAOM (C13), or uploaded directly in the form.

Fall Quarterly and Final Sign-off Forms

The evaluation forms are for logging your hours QUARTERLY or to FINALIZE the project.   

If you're project is going to continue into the next quarter, you'll select ongoing project.   For example, you're doing a Didactic Preceptorship in Acupuncture Techniques for 2 quarters, you'll select ONGOING PROJECT and submit the Evaluation each quarter.    

If you are completing your project, you'll select completed project and submit the form.

Once you click submit, the DAOM Office will review the form, and send it to all parties via DocuSign (https://account.docusign.com/#/username) for signatures.  Remember to submit all your deliverable for your project.  Your deliverables should be uploaded to your student folder, i.e. John Smith DAOM (C13), for final review.

Clinical Hours Tracking Forms

We've been slowly adding to your student folders (i.e. "John Smith DAOM (C13)"), newly developed Clinical Tracking Forms, titled "_DAOM Clinic Hours Tracker - John Smith (C13) v8-15-18."  

These forms were developed to help each of us to track which Mentorships, Preceptorships, and Service Learning projects have been started and completed.  It also will include other Clinical Hours logged, such as Clinical Rounds and Medical Presentations.  

Below is a sample of what the form looks like, including your total hours.  Please review this regularly so that your hours are appropriately logged.
