How can you request for Equitable Relief?
Write a letter to SSA explaining that you received misinformation from a federal employee.
You can call 800-772-1213 to find the address of your local SSA office or visit
www.ssa.gov.
In the letter, be as specific as you can. Including:
- Dates/ times you spoke with the federal employee
- The employees name (if possible)
- Describe the outcome of the conversation
- State whether you want coverage moving forward, or retroactive coverage and/or the elimination of your Part B penalty
*If you are granted retroactive coverage, you will have to pay premiums back to the time your coverage begins.
It is always a good idea to keep copies of the documents you send to SSA and follow up with your local office a month after you submit your letter.
