Message of Hope

Happy December, and Merry Christmas! This year has been filled with joyous moments that shine a light on the ways that God has been at work through Esperanza Health Center. We were able to provide care and resources for expectant mothers and newborn patients through our Cada Madre Fund, and our new Contigo program has paired bilingual doulas with uninsured patients, providing much-needed support together with Esperanza’s OB/GYN team. Through Dr. Daniel Chen’s Lifestyle Medicine Class, we’ve partnered with patients in learning new approaches to wellness, such as learning a new whole food plant-based diet and incorporating physical activity. Celebrating our HRSA Quality Badge Achievements!

Esperanza Health Center was recently awarded four new Health Resources and Services Administration (HRSA) Community Health Quality Recognition (CHQR) badges for 2023, including the Health Center Quality Leader - Bronze badge, for ranking in the top 21-30% of all health centers nationally in performance on a range of key clinical quality measures. Esperanza also received three other CHQR badges: COVID-19 Public Health Champion, Addressing Social Risk Factors, and Advancing HIT for Quality. The CHQR badges acknowledge health center programs that have reached significant milestones in quality improvement. To learn more about what each CHQR badge represents, please visit HRSA’s website. In addition, Esperanza received renewed recognition from the National Council of Quality Assurance as a Patient-Centered Medical Home (PCMH) for all three of our sites, reflecting Esperanza’s commitment to a model of care that puts patients and families at the forefront of care.

Said Tim Leaman, MD, Chief Medical Officer: "Ensuring that our patients are experiencing exceptional clinical care is central to our work at Esperanza. These badges recognize the quality of Esperanza’s work, the outcomes our patients are experiencing, and are a cause to celebrate and to honor the outstanding work of our staff throughout multiple departments.”

World AIDS Day Recap

On December 1, staff at Esperanza Health Center celebrated World AIDS Day. This day serves as a day to remember our family, friends, patients, and community members who have suffered due to AIDS/HIV. While acknowledging the strides in treatment, we continue to pray for a cure, fair access to life-saving treatments, and equality for all.

Patient Spotlight

Powerful P's of God

by Christy L. Concepcion

Prayer is a time set aside for a personal relationship with God almighty.

This will be the foundation of your relationship and should never be taken lightly.

Press through the pressure of Satan's attacks that will happen through your life.

These are the trials and tribulations that will set the path for your righteous rise.

Push past the things that get in the way of the plans that God has for you.

He does not have plans to harm you but knows every desire that you pursue.

Persevere through all aspects of time and obstacles to achieve your goal.

Make sure you read the Bible to help fuel the encouragement of your soul.

Peace is what will happen beyond your knowledge or understanding.

Always remember all the glory goes to God because his will did your planning.
