Happy New Year! We're pleased to share some good news that affects Cigna HealthcareSM Flexible Choice Dental, Vision & Hearing (DVH) applicants.
Effective immediately, we can now accept other forms of identification (ID), such as government-issued immigration documentation, for non-U.S. Citizens* with legal immigration status.
This change provides more members of your community the opportunity to obtain DVH coverage for themselves and their families.
Submitting other forms of customer ID
Support documentation for other forms of ID is required, but simple to submit during the application process:
Where:

Express App: On the Final Steps page, select "Other" for the question, "Which form of Customer Identification would you like to provide?"

You will not be able to fill in the SSN/ITIN field if submitting other forms of ID.
- You'll be directed to submit support documentation.

On a paper application: Leave the SSN/ITIN field blank. You'll need to provide support documents for other forms of ID per the below.
What: Other ID documentation types and form numbers include the Permanent Resident Card (AKA "Green Card"; I-551), Reentry Permit (I-327) and Refugee Travel Document (I-571).

How: Submit support documents for Other IDs via:

the document upload section of the Final Steps page (if applying via Express App).
- Select "Yes" when asked "Would you like to upload any documents electronically to your application?"
