New and improved ACP focuses on preparing patients and surrogate decision makers for communication and medical decision making!! Check out these podcasts @GeriPal and @Curbsiders to hear more.
PREPARE Increases ACP Engagement Among Older Adults with Cognitive Impairment and Their Caregivers

  • English and Spanish speaking older adults with cognitive impairment and their caregivers were able to engage in ACP with the PREPARE website and easy-to-read advance directives

  • PREPARE plus an easy-to-read AD increased ACP documentation and engagement among diverse older adults with cancer more than an AD alone. PREPARE may help to decrease ACP disparities among patients with cancer.
A New Guide Demonstrates the Synergy Between PREPARE and Respecting Choices® 
Respecting Choices and PREPARE are the only ACP organizations that have been approved as Evidence-Based Programs through the Title III-D Program of the Older Americans Act.
