Extremely Vulnerable Determination Form Released

As recently reported, Governor DeSantis has expanded COVID-19 vaccine eligibility Floridians under 65 deemed extremely medically vulnerable by a physician. CLICK HERE to download the form required to confirm the determination of extreme vulnerability - just released by the state today. Eligible residents as of today include:

  • Long-term care facility residents and staff;
  • Persons 65 years of age and older;
  • Persons under 65 deemed extremely medically vulnerable by a physician*;
  • Health care personnel with direct patient contact;
  • K-12 school employees 50 years of age and older;
  • Sworn law enforcement officers 50 years of age and older; and
  • Firefighters 50 years of age and older
  • * Vaccines for this group may only be administered by any Florida licensed physician, advanced practice registered nurse, or pharmacist.

While no information has been provided as to when physicians can expect to obtain vaccine to provide to their patients, please make sure you enroll with Florida Shots immediately. The latest updates in general can be located at the Florida Department of Health's COVID-19 Response Website
