SRC PARTICIPANT AGREEMENT

AND RELEASE FORM

Dear SRC Participants,


Each January, we provide an updated Participant Agreement and Release Form for SRC members to fill out. We've seen an increase in new people joining our exercise classes, and we want to ensure we have everyone's contact information on file in case of an emergency.


Attached is a copy of the Participant Agreement and Release Form - just click the GREEN button below to download it. Copies of this form will also be available at the Senior Resource Center each day during exercise classes, quilting workshops, String Band rehearsals, coffee hour and wellness day programs.


We ask that you fill out the forms and return them to us either by email or by handing them to either Lisa, Claire or Dee while you're at SRC. We need 100% participation and appreciate you cooperation.

Patriots Path: Palmer Park to Old Route 24 is a 7.5 mile(we'll go as far as we can!) moderately trafficked out and back trail located near Long Valley that features a river and is rated as moderate. The route takes you on a variety of terrain along the way, and takes you through the Chester Furnace Historic site.

FIRST OF A SERIES

TOPIC: “UPDATED SCAMS & FRAUDS”

Including email & text scams, romance scams, phone scams & fraud, bitcoin scams, skimming/gas station credit card frauds.


Presented by: Detective Matthew Bohn,

                         Chester Police Department

 

Detective Bohn attended Hopatcong High School and graduated in 1998. He then attended County College of Morris and obtained an associate’s degree in Criminal Justice. In 2002 Det. Bohn was hired by the Hopatcong Borough Police Department where he remained until 2009. He then transferred to the Chester Police Department (Chester Township originally). He worked as a Patrol Officer and a senior supervising officer until 2019 when he was made Chester Police Department’s first full time (SRO) School Resource Officer. As an SRO Det. Bohn was in charge of the safety and security of the Staff/Students of Bragg and Dickerson Elementary Schools. He also was (LEAD) certified, Law Enforcement Against Drugs and taught the 6th Grade middle school students the dangers of drug and alcohol use. In 2023 Det. Bohn was promoted to Detective and is currently assigned to the Chester Police Department Detective Bureau where he works on Fraud and Identity Theft cases under Lt./Det. Cavanagh. Detective Bohn is set to retire in July of 2027.
