Common Questions & Answers

In this issue, we’re focusing on responding to some common questions we have been receiving from physicians.

 

We have also created a Family Physician Plus Working Group to help us work through issues and advise on the simplest approach to implementing FM+.

How often are visits required for a patient to be considered part of a panel?

We are happy to report that there will be no requirement for a recent visit on file to be able to claim a patient as part of your age-based panel payment. In some jurisdictions, we have seen a rule in place for this. We anticipate a recent visit requirement to claim the disease/complexity portion of the panel payment, as is the case now for CCM tariffs.

 

We posted an overview of the age and disease panel payments here. We anticipate sharing the complete requirements for this in the next week or two.

 

How long do I have to submit my final CCM annual claim and transition to FM+?

We will be providing guidance in the next few weeks on how to complete your annual claim for Comprehensive Care Management (CCM) and start claiming for FM+.

 

We understand different physicians bill CCM at different times of the year. With FM+ starting April 1, 2024, we will ensure you receive clear guidance on claiming your final CCM payment.

 

For any patient that you claim your annual CCM payment for in January, February, or March, you should proceed with claiming it again on their anniversary date and by no later than March 31, 2024.

 

Can I bill for patients on my panel from a Personal Care Home?

Yes. Like all FM+ patients, PCH patients will need to be enrolled. Guidance will be provided if you need help.

 

Do I need an EMR to participate in FM+?

Yes, you must use an EMR in your practice to participate in FM+, which was already a requirement to claim CCM tariffs. The services must be documented in the EMR and securely communicated to Manitoba Health via data extracts compatible with Manitoba Health's information system.

 

I currently claim CDM tariffs. Are those disappearing on on April 1, 2024?

Chronic Disease Management (CDM) tariffs will sunset during this Agreement, but not until March 31, 2027. Any physician claiming CDM tariffs now can continue to do so for the time being. However, we strongly encourage physicians to consider transitioning sooner than later to FM+.
