Focus of the Week - Community
Community Health at the Intersection with Criminal Justice
For nearly 20 years, FMCH has been engaged in work focusing on the health of criminal justice-involved populations. The U.S. incarcerates more people than any country in the world, and Black, Indigenous and People of Color are severely over-represented as are people with low income. Individuals with serious mental illness, substance use disorders and veterans are also over-represented. Once incarcerated, people often cycle through prison and jail systems with recidivism rates estimated at 75% over five years. Barriers to addressing social determinants of health following release are well established, leading to high risk of death and disability.

From 1998-2013, the Commonwealth Medicine division of the medical school provided comprehensive mental and medical health care, with FMCH as a partner. During and since that time, the department’s role has shifted away from direct care and has focused more on research and education. Warren Ferguson, MD, Senior Vice Chair, founded the Academic and Health Policy Conference on Correctional Health and the Academic Consortium on Criminal Justice Health, widely recognized as critical dissemination and organizing vehicles for academic health centers and faculty with a shared mission to improve health outcomes of justice-involved populations and to reform mass incarceration and justice disparities.

On April 8-10, 2021, the annual ACCJH Conference took place virtually with 175 peer-reviewed presentations and academic posters presented virtually. Over 400 individuals from five countries and 35 states, the District of Columbia and Puerto Rico attended the conference. In addition, 64 students, early career investigators, community-based and criminal justice-based scholars were awarded scholarships to attend. Presenting at the conference from the UMass community were:
  • Ekaterina Pivovarova, PhD, Assistant Professor of Family Medicine and Community Health: Perspectives on the causes of addiction and treatment history among persons who use heroin.
  • Jesse Joseph Feierabend-Peters, MS4 and Warren Ferguson, MD, Professor of Family Medicine and Community Health: Coming Home: Understanding Navigator Best Practices to Assist Formerly Incarcerated Persons with health and human service needs.

From Commonwealth Medicine:
  • Audrey Smolkin, Director, Child and Family Policy, Instructor of Pediatrics: Racism, Trauma and Resilience: Transforming Systems and Practice for Youth of Color.
  • Dyana Nickl, Executive Director, Health and Criminal Justice Program and Assistant Professor, Graduate School of Nursing and Meaghan Dupuis, Senior Director of Operations, Health and Criminal Justice Program: Multi-agency collaboration to improve Covid-19 vaccine compliance.
  • Katherine London, Principle, Health Law & Policy: Massachusetts demonstration: Medicaid supports for justice-involved individuals with behavioral health needs.
Announcements

CIPC Course and Recruitment Announcement
The UMass Medical School Center for Integrated Primary Care (CIPC) is currently developing a new course entitled Certificate in Primary Care Behavioral Health for Primary Care Providers, an online, asynchronous training on key areas of knowledge to assist PCPs in providing more robust mental health care services in primary care and to do so collaboratively on primary care teams that include behavioral health providers. There will be a Course Co-Director Position open and would like to encourage people to apply.

FHCW Wednesday Cultural Rounds
The residents at FHCW host the event each Wednesday from 5 – 5:30pm. We welcome a member of the community to talk about their culture and the ways it can impact their health and the care they receive. The sessions are informal and involve a robust Q&A. Faculty Presentations

Hugh Silk, MD, MPH, FAAFP virtually presented at the National Oral Health Conference on April 16-17. Seminar entitled: Don't Reinvent the Wheel: Best Practices for Improving Oral Health for Pregnant Women and Infants; and a roundtable entitled: Increasing and Improving Oral Health Curricula in Health Schools – Here’s How! (the latter based on work with Judy Savageau, MPH and Kate Sullivan).
Department Member Recognition
 
Jim Ledwith, MD has a new role as an opioid management consultant for UMass Memorial Health Care, specifically tasked with assisting individual physicians and practice groups with establishing systems for safe opioid prescribing and guiding the transition of care when patients on opioid therapy lose their physician to retirement or relocation. He is also a physician consultant for MCSTAP and encourages all health care providers working with patients challenging pain or substance use to make use of the free consultation service for management advice and resources. www.MCSTAP.com

COVID Update
  • World 141,352,311 cases; 18.2M active; and 3,025,234 deaths.
  • US 32.3M cases and 580,756 deaths.
  • Massachusetts COVID cases 630,206; Active cases 34,682; hospitalized 693 with 169 in ICU and 99 intubated; 17,110 confirmed deaths.
  • State test positivity 1.97%.
  • Worcester (Stop the Spread) test positivity has increased to 9.8%.
  • UMassMemorial has 47 inpatients (15 in ICU).

Vaccines and COVID News
  • Johnson and Johnson on pause for evaluation of connection with rare but serious blood clots.
  • UMass will continue to reach out to diverse populations to deliver vaccinations to improve the health equity gap. 
  • UMass continues to vaccinate using Pfizer and Moderna.
  • According to the CDC, 5800 fully vaccinated people have been diagnosed with COVID. This represents only a very small portion of those who have been vaccinated. However it reinforces the need for continued vigilance after vaccination: Wear a mask, keep social distancing, wash your hands, and avoid large crowds.

The Caring for the Caregiver Program provides resources such as the Caregiver Support Line (508-334-HELP) and The Employee Assistance Program (EAP) which offers a variety of services and supports (including virtual peer support and wellness tips.
