UPDATE: FORTE Patient Trifold Brochures

The FORTE Patient Trifold Brochure is a resource available to NRG-CC005 sites to contact patients about this trial. Patient materials have been updated to reflect the change in eligibility guidelines that allows patients between 45 and 70 years to enroll in FORTE. This brochure can be mailed with introductory information about FORTE to patients to educate them before a phone call. They can also be placed in clinics as reading material, pinned to bulletin boards, included with colonoscopy prep instructions and more.

Request physical copies from the NRG Oncology office in Pittsburgh by emailing forte@nrgoncology.org. Please include the number of brochures and a mailing address for your site in this email.
