Volunteers Needed
We are seeking 100 more volunteers to participate in our NO COST fundraiser, and you could win prizes just for participating (see more details below). From now until July 5 the Forever Parks Foundation will receive $2 for every opinion survey completed during this special program, up to $15,000.
You can register now to complete 75 product review opinion surveys (max 50 per day), and participants have a chance to win prizes in a raffle. Participants must be 18 years old and up.

Check the consent box to share your contact information with Forever Parks Foundation so that your surveys will count toward our campaign. Don't worry, we will not share your information and will only use it to say "thanks" and enter your name into our incentive drawing at the end of the campaign. Make sure to select Forever Parks Foundation as the organization you support and you may also elect to represent one of the following teams with your support.

Participants completing 20 surveys will be entered into our drawing for door prizes and participants completing 75 surveys will be entered into a drawing for 2 chances to win a $100 VISA gift card.
