As you begin to make your transition to Fountain Valley School for the 2025-26 academic year, please be on the lookout for bi-weekly emails this summer to prepare you for the academic school year; the communications go out every week beginning in August.

Things to Know

Mandatory Health Forms

Due July 31

The Fountain Valley School Wellness Center serves as a campus hub, providing both healthcare and counseling services to students and faculty who need them. All required health forms linked below should be completed, signed by a physician, and submitted via the secure, web-based SchoolDoc portal by no later than July 31:



If applicable, families should also upload the following, completed by a doctor, to SchoolDoc:



Completed forms should be submitted electronically via Fountain Valley's secure SchoolDoc portal before the July 31 deadline. Families can expect to receive a verification email from SchoolDoc to begin the submission process. The verification email will arrive in the first week of July 2025. Please be sure to complete all online health forms and authorizations before reporting to campus for Registration/Orientation. Students will not be allowed to participate in campus activities, including athletics, without them. All forms must be submitted prior to Registration/Orientation in August. Questions regarding the Health Forms can be addressed to nurses@fvs.edu.


*Note: All students must bring reported medications/supplements to the Wellness Center during Registration/Orientation. Medications/supplements are NOT permitted in student rooms.

Physical Examinations Provided by UCHealth

(For local families)

Did you know that UCHealth offers walk-in or by appointment sports physicals at its urgent care and primary care sports medicine clinics? Appointment preference is given to any athlete attending a UCHealth partner school, like Fountain Valley! Physical examinations are encouraged during the summer months for the upcoming school year. Below are several healthcare facilities within close proximity to FVS for day students.

Families with Medical Insurance

Parent/Guardian confirms that their student's insurance will cover routine as well as emergency care in Colorado Springs, Colorado.


Domestic ParentsIf you do not reside in Colorado Springs, please ensure your insurance plan provides coverage in this area. Some providers may consider care in Colorado Springs “out of network” and be subject to different rules. It is your responsibility to upload any special requirements for coverage into SchoolDoc.


International Parents—Health care and medical insurance in the United States is expensive. Clinics and pharmacies will only accept insurance that includes a U.S. billing address and phone number. If your policy does not provide both, hospitals, clinics, and pharmacies may still provide treatment or medications, but services will be billed as if your student is uninsured. You will be responsible for all medical expenses incurred. Do not purchase medical insurance for your student unless it includes a U.S. phone number and billing address.


All ParentsValid medical insurance is required for every student for the full academic year.

Families without Medical Insurance

We recommend ISM Insurance, a third-party provider. Please choose one option: purchase it, print the receipt, and include it with your registration paperwork.
