Dear HMIS Provider,


This is an informational message to provide guidance for HMIS Interim Housing (IH) Service Providers regarding recent changes to IH programs in HMIS.  


As part of the LAHSA Interim Housing (IH) RFP process for Fiscal Year 2025-2026 (FY 25-26), the Adult IH portfolio has gone through changes; projects were reprocured, consolidated, or renamed in HMIS. This is part of an effort to streamline shelter services, reduce the administrative burden on agencies, and improve tracking and reporting. Consequently, for reprocured projects, new HMIS program IDs were set up under a new agency; for consolidated projects, two or more individual HMIS program IDs were merged into a new HMIS program ID; while other projects were only re-named in HMIS. See the list of all projects along with the “HMIS Action” taken on column D: FY 25 Adult IH Sites – Program Mapping List for program level details of the changes.


In the guidance below, FY24-25 programs are referred to below as “old” programs and FY25-26 programs are referred to as “new” programs.

HMIS Action

  • Active clients from the old HMIS programs that were reprocured or consolidated were bulk exited successfully on 7/1/25. This includes exits from both their program enrollment and unit assignments.

  • The new HMIS programs’ unit/bed inventory set up is ongoing until 7/15/25.  
  • status - in progress

Provider Action

  • Enroll participants into new HMIS Programs: Enroll all participants that were active as of 7/1/25 and bulk exited from the old programs into the new programs in HMIS. To confirm the new program, refer to the FY 25 Adult IH Sites – Program Mapping List, identify your agency and the column labeled “FY25-26 HMIS Program ID.”
  • All clients should be enrolled into their new programs as soon as possible by 7/15/25 at the latest.

 

  • Assign participants into new HMIS Programs: Assign enrolled clients to the appropriate unit/bed in the new program with the start date of 7/1/25. The historical data in the old program will remain and be used to report uninterrupted length of stay.
  • If the unit/bed for the new HMIS Program has not been set up yet, please track your unit/bed assignments manually until these are set up by team LAHSA (before 7/15/25). Agencies can use this template: Unit Assignment Template - Interim Housing Inventory Module.

 

  • Ensure accurate exit data in old programs: For clients that should have exited but were still enrolled in the old program when they were bulk exited on 7/1/25, this data entry lag should be corrected asap in HMIS. The client should be exited to reflect the correct exit date to the old program with the appropriate exit destination. Maintaining accurate exit data on clients is critical to reporting and a requirement for all HMIS Participating Organizations.
