Annual Update of Medical Release and Waiver Form

Parents: Please print and complete this important form for the new calendar year. I must have this updated and completed form for each of your children, in order for them to participate in youth activities both on and off site of Calvary.

I do have several that are already turned in, please contact me if you are unsure whether you have completed yours yet.

High School Youth Group (grades 9-12)

Sunday, January 28

5:00 - 7:00 pm

We will be spending an hour Football Bowling and the other hour playing Basketball Connect Four. **No skill or experience required!

Cost: $5 per person

We will meet at 1st and Bowl in Wauwatosa. (please eat dinner before you come)

Tubing at Sunburst!

Middle School Youth Group (grades 6-8)

Sunday, February 4

11:45 am - 3:15 pm

Join in the fun for a great afternoon snow tubing!

Cost: $10 per person

We will meet at church and carpool to the hill. Please bring a lunch to eat between Sunday School and tubing.

Kids - Join me for a fun night praying and playing as we discover just how SPECIAL you are!

Adults - You are welcome to join in the fun or head off site for some adult socialization.

The high school youth have graciously volunteered their time to watch the young kids in the youth room for the duration of the meeting.

There will be fun, games, and VeggieTales!

Intergenerational Events

Saturday, February 3

9:00 - 10:00 am

Bring your “Dad” or “Father Figure” for a morning of sweets, treats, and together time.

Kids and adults of ALL AGES are welcome to join us. We will have some games and activities too.

Friday, February 9, 6:30 - 8:00pm

Join in the fun games for all ages!

There will be a series of 60-second challenges that use objects that are commonly

available around the house.

Complete the games individually or with a group!

We will also have a yummy Ice Cream Sundae Bar to reward all of our competitors!
