E-Rate Survey 2027

Dear Families,

Each year our school must conduct a survey and will use the information on this Survey to maximize our outside funding, which will help to provide a better education for your children. For example, E-Rate funding will fund part of our Internet bills and help fund improvements to our wireless network allowing more money to be used for educational programs and resources for students.

Please complete and return this survey when you receive it through FACTS. It is VERY important to return this form to us even if your income does not meet any of these criteria for the school’s survey to be considered a valid measure for the school.

ALL THE INFORMATION GATHERED IN THIS FORM WILL BE KEPT STRICTLY CONFIDENTIAL

Falcons Nest is an after-school program for students in Kindergarten through 8th grade at St. Francis. Each afternoon begins with the theme of the day, followed by competition games, all supervised by a dedicated group of teachers.

Big news: Falcons Nest is now five days a week! Wednesdays have been added starting December 1, so families now have the option of Falcon's Nest aftercare every weekday. Wednesdays are Wind Down Wednesdays — a low-stress, chill afternoon of board games, card games, and conversations — running 2:00–5:00 PM. The rest of the week runs 3:00–5:00 PM: Mondays are Team Competition, Tuesdays are STEM, Thursdays are Academic Support, and Fridays are Sports Day. Students also earn Falcon Tickets for positive choices, which they can redeem for snacks, drinks, and special rewards at the Falcon Shop.

Registration is flexible: choose only the days you need at $40 per day, or sign up for the full month for $375. Please see the attached flyer for more information, and register using the QR code on the flyer or at https://forms.gle/prswEYgKa56tceu27.

We are looking for volunteers to help in the cafeteria during school lunches. Thank you to our Home & School Association for organizing this! Please click this link to sign up. All volunteers must have completed the volunteer requirements before volunteering on campus. Please contact Mrs. Arnold with any questions regarding volunteering.

An up to date sports physical and signed Extra-Curricular Eligibility Contract must be on file with Coach Morgan. This applies to students in Grades 5-8 only. Students in Kindergarten - 4th grade who participate in clubs or sports do not need a sports physical or eligibility contract.

We encourage students to participate in multiple sports, even during the same season so practices and games are scheduled accordingly. Parents are not permitted on campus for practices. Parents can pick kids up from try outs/practices at green gate by the school field or aftercare. All visitors to campus must check in at the front office.

All families with a student participating in a Fall sport must have an active FACTS financial account. A $100 athletic fee for each sport/club a child participates in will be charged in FACTS.

Please contact Coach Morgan with any questions.
