Alzheimer's & Brain Awareness Month (ABAM)

Worldwide, 47 million people are living with Alzheimer's and other dementias. Throughout the month, the Alzheimer's Association encourages the nation to help raise awareness by   going purple, sharing stories, and taking action to #ENDALZ.

The Home Health Quality Improvement National Campaign (HHQI) offers the following related resources that you can use to help promote ABAM among your staff and patients.
National Safety Month

Preventable deaths are at an all-time high, claiming more than 146,000 lives in 2015. Poisoning by prescription drug overdose and falls are the #1 and #3 causes of all accidental death in homes and communities; however, among people over the age of 65, falls are the #1 cause of unintentional death.

HHQI offers evidence-based resources that can help you promote safe medication management and fall prevention strategies among staff and patients.

AOTMJune 2017 HHQI Agency of the Month

This month, HHQI recognizes McLean Home Care & Hospice of Simsbury, Connecticut for their dedication to improving home health care quality.

Home health agencies that are active in the HHQI National Campaign and closest to the  national, risk-adjusted ACH 20th percentile and/or the national, risk-adjusted Oral Medication Improvement 80th percentile (or better) are eligible for random selection as HHQI Agency of the Month. 

Be in the spotlight.  Nominate your agency today , and if you are selected, your organization will be prominently featured here in HHQI Inside Edition -- which reaches more than 14,000 subscribers -- and on the HHQI website. Your achievement will also be shared via HHQI's social media channels.
 
More than 4,500 participants are now enrolled in HHQI University , our free online learning platform. Nearly 40 on-demand courses focus on evidence-based practices to improve patient outcomes and quality improvement for home health and other community settings. Eight topic catalogs allow participants to locate and enroll in courses that are meaningful to the work they are doing at their organization or for their own self-learning. 

Catalo g topics include Cardiovascular, Disease Management, Hospitalizations, Medication Management, Quality Improvement, Underserved Populations, and more. There is also one course designed for home health aides and more are planned for the coming year.

All of the courses include free Nursing CEs and a few have Social Worker CEs and Physical Therapy CCUs. All certificates are available immediately.

Milestone5Florida Agency Reaches Cardio Milestone 5

More than 2,400 home health agencies are participating in HHQI's Cardio Milestones program. However, only two have achieved the final milestone to date. HHQI would like to congratulate the latest Cardio Milestone 5 agency --  First Choice Home Health, Inc. of Boynton Beach, Florida -- for this outstanding accomplishment!

If improving your patients' cardiovascular health is a priority at your agency, HHQI can provide the resources and assistance you need to make your efforts successful. Our Cardio Milestones program provides a roadmap to achieving measurable improvement in the cardiovascular health of your patient population. 

You'll automatically ac hieve Cardio Milestone 1 when you  join HHQI's Progressive Cardiovascular Learning & Action Network (CardioLAN). For 12 years, Ms. Huffman has been a best-selling author and has written about health coaching, patient engagement, establishing patient/provider partnerships, disease management, and health outcomes management. She currently fills the role of Continuing Education, Presenter, Administrator, and Faculty for The National Society of Health Coaches; and formerly was the Continuing Education Coordinator for Amedisys, Inc. (Corporate).

FREE CONTINUING EDUCATION CREDIT
This activity has been approved by the Alabama State Nurses Association (ASNA) for 1.25 hours of nursing continuing education (CE). ASNA is an accredited approver of the American Nurses Credentialing Center's Commission on Accreditation (ANCC). To receive CE credit, participants must watch the 60-minute webinar and complete an additional 15 minutes of related coursework in HHQI University.

In celebration of  Community Health Improvement Week  (June 4-10), the June 2017 issue of QIO News  looks at how the QIO Program is working to improve community health by reducing hospital  readmission s and coordinating care for Medicare beneficiaries as they move from a hospital to the home or to other community health care facilities.

IMPACT Act Data Elements Public Comments Due June 26

CMS seeks comments from stakeholders on data elements that meet the Improving Medicare Post-Acute Care Transformation Act of 2014 (IMPACT Act) domains of cognitive function and mental status; medical conditions and co-morbidities; impairments; medication reconciliation; and care preferences. The Call for Public Comment period closes on June 26. For more information, visit the   Public Comment webpage.
CMSDelaysCMS Issues Final Rule to Delay 3 Bundled Payment Models

The Centers for Medicare & Medicaid Services (CMS) has finalized a rule to delay the implementation of some  bundled payment  models.  The final rule, posted to the Federal Register on May 19, 2017, pushes back the expansion of the Comprehensive Care for Joint Replacement (CJR) Model, the Cardiac Rehabilitation Incentive Payment Model, and the care coordination models. CMS  proposed  the delay at the end of March. Read more.
DarkDaysJuly 2017 Release 'Dark Days' for the CWF Hosts

In anticipation of the July 2017 Release, the CWF Hosts will not process claims beginning Friday, June 30, 2017 through Sunday, July 2, 2017. During this period, which is commonly referred to as 'dark days,' the CWF Hosts will install the July 2017 Release, complete weekly/monthly/quarterly processing activities, and perform scheduled data center maintenance. This means Medicare Administrative Contractors (MACs) will not have access to the Health Insurance Master Record (HIMR) and Beneficiary Data Streamlining (BDS) transactions. Eligibility information in HIQA and HIQH will also not be available to providers. Read more.

MillionHearts115k Cardiovascular Events Prevented in 2012-13

Around 115,000 cardiovascular events were prevented during the national Million Hearts® initiative's first 2 years compared with what would have occurred had 2011 rates remained stable. Recent flattening or reversals in some event rate trends were observed supporting intensifying national action to prevent cardiovascular events. Learn more.
