A Client In Need
Situation: 85 year old single woman, no adult children, has been living in an assisted living community for 3 years due to cognitive decline, COPD, and mobility issues. Recent fall, fractured hip requiring hospitalization and rehabilitation with a designated discharge date forthcoming. Will need 24/7 skilled nursing home placement directly from rehab. Client's sister, power of attorney (POA) and health care proxy (HCP) lives in upper Maine 3 hours away.

  •  Bank: $12,000
  • IRA: $76,000
  •  3 Mutual Funds: $46,000
  • Condo (Currently Rented): $370,000
  • Treasury Bonds: $42,000
  • Life Insurance: $25,000 ($20,000 cash value)
  • Social Security and Pension (monthly income): $3,800
 
Recommendations and Action Items: SRC case manager work with HCP sister to help select an available skilled nursing facility ASAP. Devise a financial plan to pay for the nursing home via a combination of medicare, private pay, and Masshealth-Medicaid. Guidance on all appropriate and available legal spend downs including funeral and burial, liquidate IRA, mutual funds, life insurance, treasury bonds, and temporarily collect rents while preparing condo for sale.

SRC benefits coordinator work with sister POA to obtain all Masshealth verification and documents (including five years bank records) in preparation of any Masshealth long term care application and any use of the state pooled trust if utilized. C0ntinuous care advocacy and oversight by SRC nurse case manager, throughout client's remaining lifetime.
