Collaborative Palliative Care Recipes with Murrumbidgee PHN
Predicting patient deaths improves access to palliative care and outcomes for patients in primary care, but due to time constraints, clinical judgement of the practitioner remains the core method for identification.

To overcome this limitation, Pen CS and Murrumbidgee PHN have worked collaboratively to improve GP capacity to identify patterns and characteristics within their patient datasets, using an intelligent filtering approach.

Four recipes have been developed to assist general practitioners to identify patients who may benefit from a palliative care approach.

New Look PAT Programs - Now with image and video notifications
Pen CS PAT Programs is a free quality improvement (QI) tool for Practice Support teams to standardise QI programs across general practices.

Features of PAT Programs:
  • Direct communication from your PHN to clinicians in your region, through Topbar, at the point of care.
  • Notifications for Doctors and other practice staff.
  • Customisable with patient advice and guidance, recommended health pathways (matched to patient criteria) or link to patient information.

LGA data in PAT BI - Available soon
Local Government Areas (LGA) data will be available in PAT BI in late February 2021.

All data in the PHN PAT BI database will be reprocessed to get LGA codes.
CAT Pneumococcal Vaccination Reports - Out now
CAT now shows a new report called ‘Pneumococcal At Risk’ for all patients at risk by age and ethnicity.

The new report will also show the reason for being at risk as well as the date of the last immunisation.

The childhood immunisation report has been reviewed and now shows the 13vPCV vaccination status.

Introducing our new Account Manager
Igor joins Pen CS in the role of Account manager, to help provide support to those interested in Pen CS and their suite of products.

Igor has always worked within the health sphere, working as an Allied health Professional, a practice manager, and a regional manager overlooking multiple general practices.

When not deep diving into data-driven improvement, Igor hones his skills in martial arts, videogames, anime, comics, pop culture and shares his love of heavy metal music with his family and new pup.

His background in health will prove useful in helping better understand our partners' unique needs and better work together with them to help them achieve their goals.
