2023 - 2024 Flu Season


To protect the health and safety of our patients and all of us, the MFA will require flu vaccinations for all employees. All staff and providers must receive their vaccination no later than December 1, 2023.

Where should we go to get the flu vaccine?


  1. Employees located in Foggy Bottom should get their vaccine from the Pharmacy in room G-201 (South Entrance) this year. Please make an appointment to get your flu shot today or visit us during employee walk-in hours on Tuesday and Thursday mornings in October from 7:30-8:30 AM.
  2. Administrative department employees can visit a GW MFA Pharmacy Flu Clinic to receive the vaccine: 

What should I do if I get my vaccine offsite?


  • If you choose to get your flu vaccine from your own provider, pharmacy, GW Hospital or other location, you will need to provide proof of vaccination to meet the requirement. Please submit a copy of the signed consent, a copy of your immunization record or a receipt no later than December 1, 2023 to vaccinerecord@mfa.gwu.edu


Providers


Providers who practice outside of the GW Medical Faculty Associates (to include GWUH) must submit an Authorization for the Release of Health Information Form along with proof of vaccination to vaccinerecord@mfa.gwu.edu. This allows the MFA to report your flu vaccination status to outside facilities.


Exemptions


Exemption request forms (see resources below for form) must be submitted to vaccinerecord@mfa.gwu.edu no later than November 14, 2023.
