Final Paperwork

Dear Summer Staff,

We are excited about this upcoming summer season! The Camp Copass Team is looking forward to serving with you this summer as thousands of kids and youth come to camp to hear about Jesus!

I have attached some forms and links that will need to be completed as soon as possible. If you have already filled out the Background check, Good Faith Contract, Commitment Form and our Mission Statement you do not need to fill out again.

Please complete the following items prior to your arrival on May 30th:

1. Background check: go to:www.Campcheckusa.com/campcopassstaff/

Click on the blue tab for “employees”

The password is: 75years

2. Good Faith Contract (sign and scan back to me at this email)

3. Commitment Form (sign and scan back to me at this email)

4. Child Protection Training: Go to: https://campcopass.com/summer-camps/child-protection-training/

When you take the test, it will ask which camp you are attending, just put “other”, and for Group Leader, put Denise Springer and the email address is denise@campcopass.com .

* After completing the Child Protection Training, please email the certificate of completion to amanda@campcopass.com *

5. You will be paid by Direct Deposit. The form for the Direct Deposit is attached. Please complete it and scan it back to me.

6. Also attached is a W4 form, an I9 form, and a general health form. These need to be completed, scanned, and emailed back to me.

7. Be sure you bring the following when you arrive:

Driver's License
Social Security
Health Insurance Card
Any other certifications you may have

If you can't scan and email back, just print and bring the paperwork with you when you come for orientation. Let me know if this is what you are doing so I won't be looking for it prior to your arrival.

Also attached to this email is a list of “What to Bring.” This is important as you prepare and pack to come here!
