Massage Therapists are now able to apply for a Level 1 Fingerprint Clearance card for initial and renewing licenses. Right now the application is only available to us via a downloadable application. They think that they will have the ability to apply online by mid April. In the meantime, we need to download the application, get a set of fingerprints and money order and mail it all in.
This is a requirement for all massage therapists going forward. So if you already have you license, you will need to have this in hand to renew.
To access the application:
- Go to the DPS fingerprint clearance card form triage site.
- Click on "Apply for a New Clearance Card"
- Click on Non-IVP
- This will download a PDF. You can fill in the PDF on a computer but must print, sign and mail it in with your money order for $67 and a set of fingerprints. (Fingerprint info below)
- On the form, make sure you use the abbreviations given on the first page of the download (picture below) and check off the last item on the list "BMT- Massage Therapist License ARS 32-4222 (picture below as well)
If you already have a Level 1 Fingerprint Clearance Card (IVP or non-IVP), you can call DPS and have them add on the Massage Therapy Board to your card. Call (602) 223-2279 and speak to one of the representatives (all have been very helpful and kind, but there is usually a wait time to reach someone) and tell them that you would like to add the Massage Therapy Board to your card and give them ARS number 32-4222 as the reason for needing to add massage to your card.
For fingerprinting, we highly recommend Arizona Livescan in Flagstaff, Mesa and Tucson. You can find all their locations on their website. Most of the locations are at mailing centers, like UPS stores. All of the locations can print the fingerprints for you digitally on a FD-258 card that you will mail in with your application and money order. The locations we have used have charged $29 for this service. In Prescott, you can get your prints done at the Sheriff's office on Gurley Street from 1-4pm on Tuesdays and Thursdays. It is $5 there but you need to provide the FD-258 card.(We can help at the Prescott campus for those).
If you are near one of their actual Livescan offices, they can offer full service processing where they will take the prints and mail them with the application for $109. If you do it yourself, it is $29 for prints, $67 for the application and money order fee and postage, for a total around $100. The locations are:

When you renew your massage license, you will have to submit a photo of the clearance card on the application (along with proof of your continuing education certificates).
If you are denied a clearance card due to offenses listed in sections B and C here, it is possible to apply for a Good Cause Exception which will take some time, so if this applies to you, please start the process as soon as you can.
