Boone Memorial Hospital developed plans to tackle the opioid epidemic in Boone County, and surrounding areas. The hospital, in partnership with Marshall Health, PROACT (Provider Response Organization for Addiction, Care and Treatment) of Huntington and Cabell Huntington Hospital received the AFA State Opioid Response: Community Innovation for Treatment Access and Retention grant. |

Award of the CHART Institute Patient Safety Scholarship |
|
Two $2,000 scholarships were awarded based on qualifications and interest in the fields of patient safety and risk management.
The 2020 recipients of the CHART Institute Patient Safety Scholarship are Heather Capluzzi, Director of Quality and Patient Safety at Ira Davenport Memorial Hospital, and Kayla Lee, Director of Operational Excellence at ACMH Hospital.
The Institute appreciates member interest in our scholarship program. The applications for 2021 are due by October 31, 2021.

Perinatal Safety Collaborative Playbook |

In January 2021 the Perinatal Safety Collaborative team shared their 14-month journey, introduced the Perinatal Safety Playbook and discussed member opportunities and expectations for the next 18-months. The presenters included:

The playbook is a result of the 14-month collaborative of six CHART hospital teams addressing the safety issues of obstetrical hemorrhage and hypertension. The playbook is specifically designed to provide key components of care for hemorrhage and hypertension plus a toolkit to facilitate implementation at all CHART hospitals with perinatal units.

Infant and Pediatric Simulators Available to Hone Skills |
|
CHART Institute offers two infant simulators and a pediatric simulator as a benefit of membership.
The Laerdal SimNewB and Nasco CHARLIE Neonatal Resuscitation simulators allow providers to practice neonatal skills and procedures within an environment designed for teamwork and communication with the goal of sharpening real-world clinical and decision-making skills. Proactive Claims Analysis |
|
CHART Institute's goal is to establish an ongoing partnership with your hospital team by proactively identifying risk and supporting the implementation of sustained process improvements designed to minimize patient harm and reduce organizational professional liability exposures.
Through the Proactive Claims Analysis process, CHART Institute will annually review claims data and identify outliers to determine which cases require further analysis. Predetermined frequency, severity, frequency of severity, and loss-ratio thresholds will be used to proactively identify CHART Members with potential opportunities for improvement in claims management and risk mitigation strategies. When established thresholds are triggered, the member will be notified. CHART Institute will collaborate with the member to analyze available information and determine the appropriate method for conducting the detailed analysis. After completion of the detailed analysis, an Institute member will provide a summary report with recommendations for risk mitigation and process improvement strategies including resources and tools for successful implementation. Upon issuance of the report, an Institute team member will collaborate with the hospital team member to ensure the successful implementation and sustainability of the risk mitigation and improvement strategies, develop appropriate performance metrics and establish scheduled, ongoing review and oversight of performance metrics.

CHART offers members information on proactive steps to prepare themselves against an inevitable wave of future COVID-19-related claims, both direct and indirect. Facilitating and managing change initiatives that impact multiple departments and/or disciplines can present a risk and quality management challenge. When changes are made to systems, processes, policies and/or procedures, the potential for long-term success can be improved by completing an impact analysis. Also known as a proactive risk assessment, an impact analysis should identify and assess potential risks by determining how the changes might impact the work of all stakeholders and maintaining open channels of communication across the organization. |

Risks in Long Term Care - Part Two |

The nature of long term care, in whatever form it is provided, makes it extremely challenging. Unlike acute care facilities, Long Term Care (LTC) facilities are the resident's home and, depending on the services provided, the resident's dignity and autonomy must be respected. LTC facilities do not have the luxury of telling residents what to do in the same way that acute care facilities do. Additionally, the population tends to be older adults. The risk of death or debilitating injury due to an adverse event is much greater than in the general population. This article is Part 2 of a two-part series on risks inherent in long term care. Although there are myriad risks in long term care, this article discusses risks that must be addressed in any LTC risk management program. |
