Hazards in the home can often be identified and moved or adjusted with little time and effort, but with huge payoffs in preventing a fall.
  • Are there any burned out light bulbs?
  • Do we have any loose carpet?
  • Is a chair or step stool unsturdy?
  • Are the stairs and floor clear of objects?
  • Do we have the correct handrails in place and are they functioning well?
Medication mismanagement continues to be one of the greatest contributors to falls, hospitalizations and hospital readmissions. A simple medication record form, like the one found below, can be a valuable step in reducing your risk of falls and ensuring proper usage.
