Flip the Pharmacy Change Package for January 2022 Hypertension Progression

The change package is an implementation guide which is intended to help pharmacies improve their workflows, increase meaningful patient care experiences, and understand documenting eCare Plans.   During the month of January, the focus is ongoing follow-up and monitoring which are particularly important for managing patients with hypertension. Here are the main areas to concentrate on: Enhance the role of non-pharmacist support staff within the pharmacy Collect and document blood pressure measurements Document and submit an eCare plan for patients with hypertension, including documenting their blood pressure reading Continue identifying patients who are nonadherent and would benefit from being enrolled into medication synchronization Click here or the image to check it out.
