ICYMI - Change Package for Domain One:
Leveraging the Appointment-Based Model
The Appointment-Based Model (ABM) is built on allowing patient evaluation, care coordination, and medication use support services to be efficiently layered alongside the Medication Synchronization process. Workflow Tools in this domain will help you begin transitioning to the ABM, which is the first step towards transforming traditional pharmacy practice from reactive to proactive, long-term management of patients.
