Flip the Pharmacy
Application Period for Cohort 4
Opens May 12, 2022

The Flip the Pharmacy Coordinating Center is excited to announce the application period for Year 4 of the Flip the Pharmacy program (Cohort 4) opens on Thursday, May 12, 2022.

This will be the fourth time local, community-based pharmacies will be selected to receive two years of Flip the Pharmacy coaching support aimed to transform their pharmacy practice.
 
Practice transformation has been more evident than ever and so is the role of local practice transformation leaders to help pharmacies along the way. In order to help prepare candidates, the Flip the Pharmacy Coordinating Center is offering a monthly webinar for the next 3 months leading up to the application deadline. Anyone interested in being a Flip the Pharmacy Team Lead for a group of coaches and pharmacies during Cohort 4, should attend the Flip the Pharmacy application webinar series.
 
Each webinar will be different and there will be reserved time for questions and answers.

Flip the Pharmacy Change Package for
May 2022
Opioid Progression
This month, we shift gears away from the Hypertension Progression and start the Opioid Progression. This Change Package is focused on helping pharmacy staff acclimate to documenting care provided to patients taking opioid medications.

Whether you're following along with Flip the Pharmacy or not, there are several significant reasons why you should focus on documenting care plans for patients receiving an opioid prescription. Click here to listen to the podcast Opioid Stewardship: A 3-tiered Approach to Safety.
 
You'll learn why opioid stewardship should be a critical component of your pharmacy workflow (even with everything else you have going on). Joe Moose from Moose Pharmacy and attorney Shawn Parker discuss best practices for opioid safety.

STEP ONE: Identify patients who have an opioid prescription
GOAL: Focus on patients within the appointment-based model receiving medication synchronization.

STEP TWO: Determine the Morphine Milligram Equivalents (MME) for each patient by utilizing the CDC MME Calculator, your prescription drug monitoring program (PDMP), and/or features of your pharmacy management system.

STEP THREE: Prior to dispensing, check the PDMP for fill history 

Knowing this information will help accomplish the ultimate goal of this Domain, which is building the framework for safe opioid practices and protecting the pharmacy.

Taylor Nease was reviewing her pharmacy’s delivery schedule when she noticed her driver was trekking to one patient’s address multiple times a week. Upon further investigation, Taylor discovered that this patient was taking 15 different medications, each with varying refill dates and poor adherence rates. Without delay, she contacted the patient to align her refills on a single delivery day and enroll her in the pharmacy’s adherence packaging program. Within months, the patient’s adherence rate surged, her clinical picture improved, and the driver could streamline his delivery route.
