Hall of Fame football coach Woody Hayes coined the phrase “You Win with People” in his book of the same title from 1975. Hayes was infamous for outworking the competition, but it wasn’t until he learned that team success only comes from involving others and getting them to see your vision and embrace your work ethic.
 
So, what does a football coach from Ohio have to do with a Flip the Pharmacy team from bordering Pennsylvania? Both found success by involving others. Team Pennsylvania was recently recognized with the Flip the Pharmacy Team of the Year Award.

Change Packages & Best Practices

The Flip the Pharmacy Coordinating Center has created multiple, monthly change packages.
If you are participating in Flip the Pharmacy Cohort 1 or Cohort 2, then this is your Month 2 Change Package on Diabetes and Social Determinants of Health.

This month's innovations are:
  1. Discuss blood glucose and A1C goals with your patients with diabetes, and
  2. Identify and address gaps in care for you patients with diabetes.

You can proactively use the Diabetes Checklist to help with clinical considerations related to care gaps and ideas for workflow and documentation. Or, refer to the ideas for addressing care gaps by incorporating MTM and EQuIPP into your pharmacy’s workflow.
