February 19, 2021 Dear Forest Creek Residents, Family Members and Responsible Parties,   We've had much to celebrate this week. Many residents enjoyed festive treats and personalized cards as part of Valentine's Day celebrations, and who could forget about Fat Tuesday? A special Mardi Gras-themed snack cart made its rounds, offering beads and refreshments to residents to mix up their week. Hope you all have a wonderful weekend ahead!

COVID-19 Information

Vaccine Update The federal, state and local governments continue to adjust vaccine eligibility based on available supplies. We encourage you to call the county's main COVID-19 hotline, 858-2929, to learn more. You can fill out an online form to see if you are eligible for the COVID-19 vaccine.

Ongoing Safety Precautions   We are not letting our guard down! We continue to adhere to safety precautions to protect and care for residents and staff on campus.

Visiting Our visitation policy remains the same. One family member is permitted to visit each tenant. Please contact us in advance to ensure your name is on our roster, and be sure to enter the facility through the main entrance on 2700 N. Forest Road.
