Attention Forest Creek Visitors

Please help us keep everyone safe! We have had a number of guests who are not complying with the regulations we are required to follow from the Department of Health. These rules are for the safety of your loved one, all our residents and staff, and you.
Quick reminders for Forest Creek guests:

  • All guests stop at front desk for screening upon arrival.
  • Please wear a mask
  • Wash hands and practice social distancing (at least 6 feet).
You are also welcome to schedule a Skype visit by contacting Holly Steeg at hsteeg@weinbergcampus.org.
