Forms 1099 Mailing

QualChoice will mail all IRS Forms 1099 by January 31st. If you did not update your W-9 in 2018, please fax a completed, signed and dated IRS Form W-9 to the QualChoice Finance Department at 501.707.6728.
 

Revised IRS Forms W-9 must be received by QualChoice no later than the end of February, 2019.

Reminder: Download Supplemental Tax Reporting Data
For help in completing their tax forms, employers can download a spreadsheet of enrollment data from their My Account home page at QualChoice.com. We encourage them to review the information carefully and let us know of any changes or corrections needed. For questions or concerns, they should contact their Account Service Representative.
Reporting Health Coverage on Taxes
2018 health coverage must be reported on federal tax returns. Review which forms members will receive from QualChoice for each of these plan types:
  • Employer Group
  • IQChoice Individual/Family
  • Arkansas Works
  • Health Insurance Marketplace
