Resources from the Community
Mosaic Family Health Caregiver Supports
Are you caring for a loved one and could use some extra support? Mosaic Family Health offers a variety of community programs to help you feel confident in your caregiving journey for a loved one, while at the same time caring for your most important resource: yourself. These programs are offered free of charge to any family caregiver, you do not need to be a patient at Mosaic Family Health to participate. For questions about any of these programs please contact Heather Flick, Susan Kazik, or Jackie Gonzalez at Mosaic Family Health at 920-832-2783.

Caregiver Counseling
Could you use some extra support caring for a loved one but unsure where to start? Our Caregiver Coaches work with each individual caregiver or family to best identify individual supports and resources that may improve the caregiver and their loved ones wellbeing. Through this service, the Caregiver Coach may provide education, guidance, help connect to community resources, and teach techniques and strategies for coping with behaviors and caregiving situations based on each individual caregiving situation. This includes stress management strategies and problem solving for a variety of situations. Meetings can be held in person, in home, over the phone, or virtually, and family meetings are available. Our Caregiver Coaches are also able to assist with Advance Care Planning for the caregiver or family members.

RCI REACH Program
Mosaic Family Health Caregiver Coaches are also certified to provide in home support directly to caregivers utilizing the RCI REACH Program (Rosalynn Carter Institute for Caregiving), Resources for Enhancing Alzheimer’s Caregiver Health. This unique evidence-based program brings information, 1:1 support and strategies for coping with troubling behaviors while improving emotional, social and physical health. Meetings with the caregiver are provided at home for up to 12 sessions over a 6-month period. Each meeting offers opportunities for in depth education, problem solving, stress management and support while improving caregiver confidence.
