Join presenters Paul Chiang, MD (Senior Medical and Practice Advisor, HCCI; Medical Director, Northwestern Medicine HomeCare Physicians), Tammy Browning, PA-C (Founder & President, Grace At Home), and Kera Holmes, CPC(R) (Manager, Practice Development, HCCI) as they discuss the 2024 Medicare fee schedule for home-based medical care.


They will review the coding, reimbursement, and policy impacts specific to home-based medical care as a result of the CY 2024 Medicare Physician Fee Schedule Final Rule (MPFS) and how to best respond to these changes. The presenters will also:


  • Discuss payment and documentation requirement impacts to the home and domiciliary Evaluation and Management (E/M) codes and identify alternate reimbursement opportunities.
  • Call out changes to home-based medical care management services, including Chronic Care Management, Transitional Care Management, Telehealth, Behavioral Health, and Advance Care Planning
  • Provide clarification on risk adjustment models, including Hierarchical Condition Category (HCC) Coding.
