During the event, our expert faculty will discuss functions performed by the front and back offices within a house call program and the wide spectrum of processes and technologies available to improve your operational workflow. Exclusive Early Entry and Nomination Process Opens for Second Cohort of Illinois House Call Project

To address the urgent need for more house calls in Illinois, HCCI launched the Illinois House Call Project (ILHCP) in 2022 with a select group of highly qualified Illinois medical practices, including independent entities, those affiliated with hospital systems, and even specialty groups such as dentistry, dermatology, and wound care. This first cohort is well on its way to exceeding its three-year goal.
Due to this success and the ongoing need, HCCI is now offering Illinois practices a period to be exclusively considered, or nominate another practice, for applying to the second cohort of the ILHCP that will launch in early 2024. With the addition of this cohort, the new goal of the ILHCP will be to collectively enroll 6,000 new homebound or home-limited people from 2022 to 2026.
The second cohort will also include a select group of highly qualified Illinois medical practices, including independent entities, those affiliated with hospital systems, and even specialty groups, and will also include hospice and palliative care programs.
Each selected practice will designate up to three “Home Centered Care Champions” to lead their work. HCCI will provide the Champions with expertise, guidance, and comprehensive resources, including education, consulting, tools, market analysis, marketing/PR training, and the opportunity to receive capacity-building funding.
