Attend Free Illinois Hospice and Palliative Care Event | |
|
Join your Illinois colleagues in hospice and palliative care, and others caring for people with serious illness, at The Intersection of Home-Based Primary Care and Palliative Care, the Home Centered Care Institute’s (HCCI) free learning event on Thursday, September 14, 3-5 pm CT, in Schaumburg, IL.
Our expert faculty will review real-life case examples demonstrating how to bring home-based primary care and hospice/palliative care together and will also provide valuable educational resources. In-person participants will then share best practices and lessons learned. Alongside vital networking, in-person participants will also receive beverages and light snacks.
This is the second event in HCCI’s Crossroads of Care: Managing Serious Illness in the Home series. Learn more on our new Crossroads of Care web page, which announces upcoming events. Couldn’t make the first session? Review the recording here: Bridging the Gap with Home-Based Primary Care.
