Meet Your Illinois Hospice and Palliative Care Colleagues at the Crossroads

Join your colleagues from hospice and palliative care programs, and others involved in caring for people with serious illness, from Illinois and the surrounding areas, on Tuesday, June 13, from 4 to 6pm Central, at the Hyatt Regency Schaumburg, for a FREE learning event sponsored by the Home Centered Care Institute (HCCI) entitled, Bridging the Gap with Home-Based Primary Care.

 

With the usage of value-based care models growing, the need for home-based primary and palliative care is on the rise. During the session, you will learn more about the unique benefits of each type of care, the value and opportunity of bringing them together, and how to successfully implement a complex illness management model that incorporates both services. Afterward, participants are invited to stay for an optional reception from 6 to 7:30pm, accompanied by hors d’oeuvres and cocktails, with Champions from HCCI’s Illinois House Call Project.
