Additionally, if the resident is unable to complete the above form, the Responsible Party should also complete the COVID-19 Responsible Party Consent Form, which has been previously provided and is provided again below. Verbal consent from the Responsible Party is permitted for residents who do not have physical or cognitive ability to consent (or minors). If consent is collected verbally by the community, the Responsible Party must contact the Executive Director and speak directly to them for consent. Voicemails and messages with other community staff is not acceptable.
