COVID-19 Updates / N95 Masks

Important COVID-19 Vaccination Update for Children Ages 6 Months - 4 Years Old

Community Health & Dental Care is now offering the Pfizer COVID-19 vaccine at our Coventry Mall location for children ages 6 months through 4 years old. The vaccine is available to patients and members of the community.


CHDC is also pleased to offer both Moderna and Pfizer COVID-19 vaccines at our Coventry Mall location. We offer 1st, 2nd, 3rd and booster doses for patients 5 years and older. Boosters are available 5 months after the completion of your COVID vaccine series and your vaccination card is required upon scheduled appointment.


N95 Mask and COVID-19 Rapid Testing Kit Distribution

FREE N95 Masks and COVID-19 Rapid Testing Kits are available, by request, to patients and community members at all CHDC locations. Patients will be able to request and pick up masks and testing kits at their appointment or by coming to the health center. If you are not a patient, please visit one of our four (4) health center locations to request and pick up your masks and testing kits. Masks will be limited to three (3) N95 Masks per person and Testing Kits will be limited to two (2) testing kits per person.


We are following the CDC guidelines for Covid-19 vaccinations. Protect yourself and others against Covid-19. Please continue to wear a mask, wash your hands, practice social distancing, and get vaccinated.

Barto Backpack Giveaway


CHDC hosted a Back to School Backpack Giveaway at our Barto office.

We filled 100 backpacks with basic supplies to start the school year.


We still have plenty of backpacks. You can stop by the Barto office for pick up during office hours.


To pick up a backpack, please stop by the Barto health center at 1315 Route 100 North, Barto, PA 19504. No phone calls please. See you at Barto!

3D Screening Mammograms


We have scheduled dates between September - December to have a mobile mammogram coach at CHDC's mall location. Here are the details:

Our most recent PT grad is feeling better from her sessions at CHDC, and she is ready to study abroad in France, now.

Physical therapists are movement experts who improve quality of life through prescribed exercise, hands-on care, and patient education.


Physical therapists diagnose and treat individuals of all ages, from children to people at the end of life. Many patients have injuries, disabilities, or other health conditions that need treatment. Physical therapists examine each person and then develop a treatment plan to improve their ability to move, reduce or manage pain, restore function, and prevent disability.

Cholesterol Awareness


Know your numbers:


September is National Cholesterol Education Month, making it the perfect time to take action to know your cholesterol levels – and take steps to prevent or reduce high cholesterol.


Do you know how food, family history, and medicine can affect your cholesterol? Healthy eating and physical activity are proven to lower LDL cholesterol.


High cholesterol is a major risk factor for heart disease, which is the leading cause of death in the United States. What is cholesterol? Check out this short American Heart Association® video about cholesterol. Check. Change. Control. Cholesterol.


If you have high LDL cholesterol levels, your health care team may recommend cholesterol-lowering medicine and lifestyle changes to lower your risk for heart disease and stroke.


High cholesterol has no symptoms, so many people don’t know that their cholesterol is too high. A simple blood test can check cholesterol levels.

Welcome our New Provider

Community Health & Dental Care (CHDC) welcomes new dental provider, Dr. Minyong Chen to its Pottstown health center location at 351 W. Schuylkill Road, Pottstown 19465. Dr. Chen comes to CHDC as an experienced dental provider treating patients of all ages.


Dr. Chen earned his both his Doctor of Dental Medicine from Temple University Maurice H. Kornberg School of Dentistry and Bachelor of Science Biology from Temple University in Philadelphia, PA. In addition, Dr. Chen served as a volunteer with Mission of Mercy to provide a dental day of service providing free dental care for those in need where over 1700 people were served.

Did you know childhood cancer is the number one disease killer of children and is the leading cause of death for children between the ages of 4 and 14 in the United States? ACCO is determined to change these statistics and make childhood cancer a national health priority by shaping policy, supporting research, raising awareness, and providing educational resources and programs to children with cancer, childhood cancer survivors, and their families.


Do you want to help, but aren’t sure how? Work with the ACCO to organize a Go Gold® for Kids with Cancer fundraiser! “Go Gold®” events are a fantastic way to have fun while showing your support for children fighting cancer and raising awareness of this important cause in your community.

Often people only associate obesity with food or overeating, but there is another factor which contributes to being overweight.


Did you know adults and children spend over 7 hours a day being sedentary, and this doesn’t include time spent sleeping. Many of these hours are spent sitting or laying down with a phone, tablet, or computer; watching TV; or playing video games.


This Fall, plan to take some time to go outside, go to the park, walk, or ride a bike. Find an outdoor activity or sport your family enjoys together. It only takes 30 minutes of movement each day to create a healthy habit.

Exercise, eating well, and drinking water lowers your stroke risk. Knowing where you stand from a physical exam will help you monitor your risk. transportation barriers.

Patients can schedule free shuttle service to and from any of our health center locations.

Qualified Medicare Beneficiaries Can Save Thousands

CHDC's Case Management team will help Medicare recipients navigate the Medicare registration process and understand the Medicare buy-in program which helps eligible individuals pay for certain out-of-pocket healthcare costs. Individuals do not need to be a CHDC patient to take advantage of this free service. Seniors with limited income and resources may qualify to have their Medicare Part B premium paid for, saving thousands of dollars by receiving maximum benefits from the Medicare program. Medicare beneficiaries are encouraged to call and make an appointment with our Case Management team at 610-326-9460 ext. 117
