If you are a State of Maine Employee, you need this information!
You can now be part of the Gym Membership Reimbursement Program.
Here's what you need to know:
Program Starts: July 1st, 2016
Who Qualifies: State of Maine Employees eligible for State paid benefits (full & part time)
Requirements: Must attend Champions at least 8 times per month (Champions Fitness Club tracks usage through your key tag swiping device)
Reimbursement is up to $40 per month per employee and will be disbursed in your paycheck quarterly after submitting proof of payment, usage report & the State Gym Membership Program Reimbursement Request Form to your agency's Human Resources/Payroll office.
