GaHIN Expands Access to Thousands of Organizations Through New Carequality Connection

GaHIN is pleased to announce that the organization has completed its connection to Carequality, a national health information exchange framework that is enabling care coordination for more

than 600,000 care providers, 50,000 clinics, and 4,200 hospitals. 


Benefits to Members

By implementing Carequality, GaHIN now enables its members to connect to approximately 2,000 additional acute, ambulatory, and post-acute care organizations across the state of Georgia. In addition, this connection allows members to access data from all types of healthcare organizations that leverage technology solutions from some of the most popular software vendors, including EPIC, athenahealth, eClinicalWorks, Meditech, PointClickCare, Cerner, Commonwell Health Alliance, and many others who support Carequality connections.  


While some Georgia healthcare providers may already have access to Carequality through their electronic health record (EHR) providers, many do not due to the costs and complexities associated with making these connections. As a GaHIN member, healthcare professionals can access the information they need at no additional cost.


GaHIN is already connected to eHealth Exchange, a national network that connects federal agencies and non-federal healthcare organizations so medical data can be exchanged in all 50 states. With the addition of Carequality and the direct connections GaHIN has with its members, many more Georgia healthcare providers are now able to access and exchange necessary patient data to improve care and public health.

GaHIN Draws a Big Crowd at HIMSS'24

GaHIN Executive Director Dr. Denise Hines and Program Manager Shirelle King wowed a crowd of approximately 200 people who attended the HIMSS session, “Enhancing Whole-Person Care Through Clinical and Social Data Integration.” 


GeorgiaUnify in Focus

The team walked attendees through GaHIN’s new social care integration platform, GeorgiaUnify, and how GaHIN is pioneering solutions that facilitate the integration of both clinical and social data to facilitate referrals and coordination of services for more effective whole-person care throughout Georgia. The new platform gives healthcare providers and community based service organizations the ability to exchange data in a closed loop referral process that ensures citizens get the care and services they need. 


Real-World Use Cases

King shared with attendees how GaHIN is partnering with CVS Health/Aetna for a pilot project to provide closed-loop referrals and better care coordination in rural Georgia communities with high needs. The goal is to demonstrate how closed-loop referrals can improve patient safety and satisfaction.


Attendees learned about the wide range of social determinants of health (SDOH) data that GeorgiaUnify supports:

  • Food insecurities, which correlates to higher levels of diabetes, hypertension and heart failure
  • Housing instability factors into lower treatment adherence
  • Transportation barriers result in missed appointments, delayed care and lower medication compliance


Several board members and partners as well as state and federal officials were on hand to show their support of and interest in what GaHIN is doing. GaHIN is leading the way when it comes to integrating clinical and social data to deliver more whole-person care.

Time is Running Out!

With the deadline rapidly approaching, we want to make sure your service is not interrupted. GaHIN is wrapping up its transition to a new, more powerful technology platform that will allow members to more easily exchange data with more healthcare providers and organizations.

 

Official notices and outreach efforts have been underway for many months to help our members avoid service disruptions. All members must complete their transition to the new platform well in advance of June 6, 2024 when access to the legacy platform expires.
