What's New?

Garden House residents can look forward to a new enhancement that's in the works! We are transforming one of our tub rooms into a spa lounge - view a "sneak peek" below! Decorations and finishing touches will be complete within the next week or so.

We continue to offer resident activities to promote engagement.

Residents received a special visit from a local canine friend, Remi.
Relaxing manicures, adult coloring and exercising are regular favorites.
We held a wheelchair "car wash" resulting in sparkling set of wheels.

We are pleased to offer the Independent Health Medicare Family Choice Plan to Garden House residents. Rosa Coplon Living Center residents have benefited from this program for years, and because of its success, we are expanding its availability to our assisted living residents.

What is Family Choice?

Family Choice has provided an extra level of care and support to assisted living and nursing home residents in Erie, Niagara and Genesee counties for over 15 years. The program offers an integrated team of nurse practitioners, physician assistants, medical directors, and social workers who work with residents and their families to address their healthcare needs right where they live.

How Does it Work?

Plan members are assigned a Family Choice Nurse Practitioner (NP) and a Social Worker who collaborate with Weinberg Campus staff to develop an individualized care plan that addresses the member’s specific health needs, as well as proactively identify potential issues before they become serious. The Family Choice NP keeps in close contact with the resident’s family and primary care provider, ensuring they are involved in the care planning process.

When Can My Loved One Enroll?

Garden House residents have the option to enroll in the Family Choice Independent Health Medicare Plan beginning May 1^st. After that, enrollment occurs on a rolling basis and goes into effect the first of every month.

This specialized program will allow us to continue focusing on what matters most – enhancing the quality of life for our residents by making sure they receive the healthcare services they need, when they need it. The Family Choice team serves as an additional resource to our already qualified and devoted staff and will be available 24/7 should any resident need immediate medical attention.

We are thrilled to offer this healthcare opportunity to our assisted living residents and look forward to welcoming the Family Choice team as an extension of our Weinberg Campus family.

We look forward to seeing you! In preparation for your visit, please review the following guidance:

Weekday visiting hours are: Monday - Friday, 9:30 - 10:30 a.m.; 11 a.m. - Noon; 1:30 - 2:30 p.m. and 4 - 5 p.m.

Evening hours will be available Wednesdays from 6 - 7 p.m.

Weekend hours, on designated weekend days each month, will be held from 9:30 - 10:30 a.m.; 11 a.m. - Noon; 1:30 - 2:30 p.m. and 4 - 5 p.m.

Visiting hours are one hour in length.

Please ring the doorbell at the Garden House main entrance and a staff member will greet you to begin the sign-in and screening process.

Visits will be held in resident rooms; designated visiting areas, or designated outdoor areas (weather permitting).

If your loved one is currently on a 14-day quarantine, they are not eligible for in-person visits. However, virtual visits are still available.

All visitors will be screened for COVID-19. This will include a temperature check and questions about exposure to the virus and recent travel. If you have signs or symptoms of COVID-19 or close contact with someone with COVID-19 in the prior 14 days (regardless of your vaccination status), you will not be permitted to visit in person.

Sanitize your hands when you arrive and throughout your visit.

Wear a face mask covering your nose and mouth at all times.

Individuals must maintain social distancing of at least 6 feet.

Please use designated seating in each room and please do not move seating to ensure 6 feet of social distancing.

A designated VISITOR restroom is available in the front foyer.

Please go directly to and from your loved one's room or designated visiting area. Visitors are not permitted anywhere else in the facility at any time.

Up to two individuals will be permitted to visit a resident at any one time.

Visitors under age 16 must be accompanied by an adult 18 or older.
