What Does Our Case Manager Do?
Case Managers can help with:

  • Insurance questions – change in insurance or updated cards should be given to Case Manager
  • Financial questions about when to begin the process for Medicaid or Long Term Care coverage
  • Room status, keys, TV, WiFi
  • Housekeeping
  • Maintenance
  • Dietary
  • General concerns (not of a medical nature)
  • Appointments
  • Transportation
  • Billing questions

Hanukkah Begins November 28

As you travel and visit with family and friends this holiday season, we encourage everyone to use precautions to stay safe and healthy.

Holiday Gifts

The season of giving thanks and gift giving is upon us. We know that you appreciate the hard work of all of our staff members. Thank you to our many family members who have shared notes of thanks or other items with our employees. Items such as thank you notes or food for teams and departments is always appreciated. Please remember that employees are unable to accept individual cash tips or gifts.
Reminders for Visitors
  • Visiting hours are 10 a.m. to 7 p.m., no appointment needed.

  • All guests: stop at front desk (2700 North Forest) for screening upon arrival.

  • After screening, go to Garden House and type the door code to enter.

  • Visits will take place in resident rooms our outside the front of campus.

  • Visitors must escort their loved one outside and back to their room at the end of their visit.

  • If you would like to take a resident outside, please let the nurse know first.

  • Up to two visitors at one time. Visitors under age 16 must be accompanied by someone 18 or older.

  • Please wear a mask, wash hands, and social distance (at least 6 feet).
