Advance Care Planning

Get the treatment you want when you can no longer speak for yourself.

Have you ever worried about what would happen if you could no longer speak for yourself about the treatment you want to receive? Though the idea is frightening, there are steps you can take to make this time less stressful for you and the ones close to you should such a situation arise.

Advance Care Planning (ACP) is the process by which you determine your wishes in regard to medical care and treatment including the designation of a health care representative. These wishes can be expressed through several types of legal documents.

ACP is completely voluntary, so you may choose to fill out as many of these forms as you'd like. However, the more information you provide, the more you reduce the risk of receiving treatment you don't want.

PACE of

Northeast Indiana


Clinical and social care for Allen County seniors.

Have you heard of PACE of Northeast Indiana? This program of all-inclusive care for the elderly offers both clinical and social services in one setting. Funded by Medicare and Medicaid, PACE helps people meet their health care needs in the community instead of going to a nursing home or other care facility.

Great Day Café
