Acquisition of the Year Award
and the WINNER is...
Stryker – Wright Medical
The votes were tallied and by a slim margin Stryker came away with the hardware this year in the Acquisition category. Money can’t but you everything, but 4 billion will get you an upgrade in your extremity’s portfolio and more importantly the First Golden Saw Bone Award of 2021. 

Looks like Christmas has come early for CEO Kevin Lobo and Direct of Foot and Ankle Patrick Fisher. Congrats!

Shout out to DJO and Gary Justak’s team and Pete Denove’s DX team at Arthrex for making a run at the award this year.

New Product Launch - Hardware Award
and the WINNER is...
New material technology wins out and Ossio nabs this year’s award for their innovative 4.0 Compression Screw made of their proprietary OSSIO FIBER. Good stuff! We are looking forward to seeing what comes next from this company. 

NOVA STEP made a late push with there CREED ORTHOLUCENT SCREWS, maybe their PECAPLASTY nomination next week can take home the GSA?

The year of PSI has arrived for the total ankle market. The next generation of surgeons has grown up expecting and learning how to perform replacements using PSI. The market has recognized that and adapted by launching versions of their own. Will the new platforms help to take a chunk of market share from the marketing leading Prophecy platform? Who knows…but let us know what you think by placing your vote for this year's award.


TAR INNOVATION
Nominees:
EXACTECH – VANTAGE ANKLE PSI

Exactech launched the first known competitor to Legacy Wright Medical’s Prophecy System this year. In an effort to not just reproduce what has been offered, Exactech looked to create an upgrade to their system with some differentiators to the Prophecy platform:

  1. Cut through Guides: Patient specific guides designed to align and cut in one step, no pin dances with the cut block.
  2. Preserve the Periosteum: Soft tissue offsets are built into the guides to allow for a consistent fit without disruption to the periosteum.
  3. Reproducibility and Consistency: The cutting guides are design to avoid osteophytes or the diseased joint to reduce variability caused by disease progression.

The Vantage ankle, created by the team at Duke, has a way to go before catching the market leader. Adding PSI will help them gain traction and equip their sales force with the technology needed to pitch their ankle protheses on an even playing field with the Stryker and others.
IN2BONES - ORTHOPLANIFY PSI

In2bones received FDA clearance for their PSI in September of this year, shortly before AOFAS. The Quantum Total Ankle is the newest on the market, here is a brief description of the implant:

The QUANTUM Total Ankle System is designed to provide unsurpassed implant stability due to its unique vertical, cruciate-shaped tibial stem. The talar component, available in flat and chamfer cut options, features an anatomically designed double radius of curvature. This combination is intended to improve function and longevity compared to currently available total ankle products.

Developing an ankle is one thing, adding PSI makes it a realistic option for potential surgeon users assuming the representation and implant design executes. A step in the right direction for In2Bones.
PARAGON 28 – MAVEN PSI

Paragon28, we all knew would add PSI. The question was more when and what bells and whistles would Paragon bring to the table. The MAVEN PSI for the APEX 3D Ankle uses a periosteum preserving guide with a medial contoured wrap to reinforce guide stability. Similar to the Vantage PSI, this system avoids osteophytes and uses the true contour of the patient’s bone geometry to allow for a snug fit. P28 offers a laser guided accessory called the "Fast Trac" for micro adjustments of varus valgus which can be used with MAVEN or aid when using standard instrumentation without PSI.

CurveBeam, the market leader in Weight Bearing CT Systems, includes the MAVEN PSI protocol. 

WBCT imaging reveals functional inversion/eversion of the ankle that is not presented on supine CT. Using WBCT to assess foot & ankle alignment in three dimensions could be instrumental in preventing osteolysis, a condition which often leads to implant failure, according to a 2019 study by Lintz et al published in Foot & Ankle International.

P28 continues to answer the bell, will an answer to the INBONE be next?

Not to be out done, Stryker recently announced their updated solution with a focus on saving time with a cut through guide of their own. Similar, but a little different from Exactech, the tibial cut through guide is designed for a metal cut block to be placed within the PSI block. In addition, it leaves two distal talar holes for those surgeons who prefer to couple their cuts using the low-profile Infinity implant. 

The prophecy system has been used in 35,000 cases over the last nine years and stood alone for a long time. Will the Cut Through Resect Guide be enough to take home another Golden Saw Bone or are the tides going to turn in the world of TAR market share?

Soft tissue in foot and ankle has long been dominated by Arthrex and that reign looks to continue. Over the next 5 years, this market seems to have the largest runway for growth for most of the other foot and ankle players. Can Arthrex remain king…or will a new player emerge?


INNOVATION IN SOFT TISSUE

Who says you can teach an old dog new tricks? Arthrex Soft Tissue makes advancements to their long standing tenodesis platform with the "tension slide" system. 

Traditional insertional tendon transfer repairs and reconstruction constructs have historically been limited since they often rely on a single point of fixation and the patient’s inherent bone quality for strength. You can now optimize your tendon repairs with Arthrex’s Tension Slide technique.

The Tension Slide technique introduces a DX cortical button on the far cortex of Arthrex’s proprietary blind tunnel technique to reliably draw the tendon to the bone socket. This technique offers users three points of fixation to allow for early weight bearing and eliminates the variability of fixation with poor bone quality. In addition, the inclusion of the cortical button substantially reduces the length of tendon required to be harvested, reducing patient morbidity with smaller incision length and soft tissue dissection.

This is one of those “I didn’t know I needed this” products, but it really makes sense and would seem like something to try for FHL/FDL repairs. Kudos Arthrex for yet another innovation in Soft Tissue.

Medline gets another GSA nomination with a device looking to take a bite out of the valuable syndesmosis market. Recent product launches into the space have failed to slow down the Arthrex train, can the Syndex make its mark?

SYNDEX™ with Constrictor® Technology is a knotless adjustable button technology indicated for syndesmosis repair. The implant features a novel, self-locking design to help prevent loss of reduction. The device demonstrates superior fixation under cyclic loading and a substantial difference in load-to-failure compared to the market leader1. UNITE Lateral Fibula plates feature specially designed holes to accommodate the lateral suture button with minimal prominence.

Medline has created its base plate and screw line over the last five years, which seems well thought out and well designed. They are now stepping up their game by offering something that can help accelerate surgeon conversions and if successful fund future innovation.
ARTELON – FLEXBAND SYSTEM

Artelon took a material with 20 years of history and created a “stretchy shoelace” to add strength to ligament repair with a “rebound effect” to act like a natural ligament. This novel material is designed to support the repair at the outset and reabsorbs over a 4 to 6 years period while maintaining mechanical strength. 

Piggy backing on Arthrex and Smith and Nephew, the Artelon team has packaged the material in kits that provide reproducible systems to address soft tissue repairs including lateral ligament recon, brevis, plantar plate, and spring ligament. Lateral ankle is huge market and Artelon is looking to address a soft tissue segment with tons of growth opportunity. Using a mechanical and biologic system that offers some differentiation than what’s currently available today makes them a massive acquisition target and a potential winner of a Golden Saw Bone.

Stryker, looking to expand upon its Sports offering, launched an interference screw system made of a resorbable material technology known as Citregen. It uses a soft thread, press fit technique to avoid tendon tears when inserting the device for fixation.

The Citrelock Tendon Fixation Device System offers surgeons:
  • A controlled and homogeneous resorption process that prevents bulk degradation and chronic inflammation.
  • Compressive strength that is comparable to cortical bone with a modulus comparable to cancellous bone.
  • Citregen contains citrate, calcium, and phosphate molecules that are inherent to the bone anatomy.
  • Material polymer structure that mimics the extracellular matrix protein network.
  • Citregen maintains structural integrity during the healing phase, while the implant is replaced by host tissue over time.

Wright Medical brought over the PEEK G-Force tenodesis system in the acquisition, however it looks like Stryker is upgrading its offering by moving to a new platform with potentially better technology. Citregen seems to be another foundational element to potentially create an entire sports platform around, we shall wait and see.
