By order of LA County Department of Health, all graduation ceremony attendees must pre-register to attend the event. Please click the link below to complete the form.

Maximum FOUR tickets/attendees are allotted per graduate. Requests for up to TWO additional tickets can be made by clicking the link in the confirmation email. While we will make every effort to accommodate requests for additional tickets, we cannot guarantee them.

THIS FORM IS DUE NO LATER THAN MAY 30, 2021 at 5:00pm PST.
No exceptions will be made for late registrations.
- LACHSA will follow the covid-19 safety guidelines set by Santa Anita Park and LA County Department of Health.
- All guests must have a reserved seat
- All attendees will be seated in box seats with members of their group. 
- Group members will attest that all members are from one household and are in-state visitors only.
- Box numbers will be assigned randomly.
- There will be no designated area for vaccinated versus non-vaccinated attendees.
- Those families requesting additional tickets will do so on a separate form.
- Only those attendees who registered through this form will be admitted to the venue.
- ALL attendees are REQUIRED to do a health check before entering the venue.
- Masks must be worn at all times.
