I hope this message finds you well. My name is Julie Sacks, and I am writing to introduce myself and the Home Centered Care Institute (HCCI). As President of the organization, I wanted to reach out because our team brought your organization to my attention as one that delivers exceptional home-based palliative care.
We realize that providing quality care while supporting this mission and reaching organizational revenue goals is more challenging than ever. To support organizations like yours in this challenging time, HCCI — in collaboration with our funding partner, Elea Institute — is offering grant support to help develop home-based palliative care capabilities. This grant is limited to up to 10 home-based palliative care practices in the greater Chicago area.
With a focus on improving care and health outcomes, reducing readmissions, and creating new program revenue streams, this grant will allow you to access support through the knowledge of experts in the field who have implemented leading home-based palliative care service programs.
This education aims to strengthen local home-based healthcare initiatives and prepare programs like yours for the future. The resources, support, and guidance enabled through this grant are designed to enhance your organization’s care model and business model.
