Grievances, Disputes and Appeals Mailing Address


Applicable To: Mercy Care Complete Care, Mercy Care DD, Mercy Care DCS CHP, Mercy Care Long Term Care and Mercy Care RBHA 


As a reminder, all Grievances, Disputes and Appeals must be sent to the street address indicated in each section of our Provider Manual under the Grievances, Disputes and Appeals sections. The street address for all lines of business is:

In order to avoid undue delays, DO NOT Send Grievances, Disputes or Appeals to our normal Claims P.O. Box for Claims. This may cause undue delays in the turnaround of these important documents.  


Provider disputes may also be filed via email at mercycareappeals@aetna.com or fax to 602-351-2300. Not only do we now have the ability to receive disputes by fax, but we can also respond back to our providers via fax, allowing you to receive faster decisions.
