Studying the association between diet-style, prescription drugs and BMI

Thank you in advance for participating in this study by sharing some data about you and your dietary habits with us. Note that you must be at least 18 years old to participate.

Assessing your diet-style. We use our online 4Leaf Survey to do that. It is a simple, 2-minute, multiple choice questionnaire at 4leafsurvey.com. After taking the survey, you will be given an opportunity to receive a complimentary custom report about your current diet-style.

( Note: Do not include contraceptives or drugs for mental-related conditions like depression or ADD.)
