We would like to thank everyone who was able to attend our Christmas party! We appreciate each and everyone of you. ROOM AND BOARD CHANGES EFFECTIVE 2020 FOR MEDICAID CLIENTS
New Room and Board rates effective: 01/01/2020:
  • $419.00 for Cost Share or higher reimbursement (SSI pending)
  • $1384.90 for SSI eligible Clients
*There are no changes to the Personal Needs allowance. The member will keep any income in excess of the Room and Board for their personal spending*
CCFFH CLAIM SUBMISSION
New rates to the CMA Service Fees for CCFFH Claims Submission effective January 2020. Please click link below for the schedule of fess.

Friendly reminder: Billing problems, it is not appropriate to call another caregiver and discuss your clients financial status. This is a violation of HIPAA.
TRANSFER REQUESTS
Reminder: Request for transfer will be done on a priority basis. Please do not request for "As Soon As Possible". Documents for transfer must be ready. RN must be present during the transfer, unless otherwise not indicated. Office is closed on the weekends and major holidays.
UNANNOUNCED VISITS
We encourage all Caregivers to check your chart for completeness. If you CTA certificate's expiration date is within 1-2 months and you are missing any documents, please notify your monitoring RN. Or you may email [email protected] or fax request to 536-7200. The assigned staff will be able to assist you.
KUDOS & COMPLAINTS
