Family Feedback Survey - Please submit by December 7, 2020
During the board of education meeting on Nov. 19, we presented recommended bell and in-person lunch schedules for second semester. We also shared a schedule we can use if we need to be fully remote or cannot offer in-person lunch service. You can access all of the proposed schedules by visiting https://bit.ly/2UXL2NY.
As part of this process, we collected student feedback on a variety of topics (e.g., teacher interactions and experiences, learning environment, etc.) that helped us identify strengths and opportunities for improvement from first semester, and enabled us to make informed decisions about our proposed plans for the second half of the school year. With this in mind, we have created this form so that you can share your thoughts on several of the same topics, and also confirm whether you want your child to participate in our hybrid or remote instructional model following winter break.
Below are a few important things to keep in mind when you are filling out this form.
1. We are asking you to please provide your child’s student ID number so that the data we receive is easier to organize, sort and search. If you do not know your child’s ID number, you can retrieve it by logging into the Home Access Center.
2. We are providing three different options for second semester – participate in the hybrid model; begin learning from home on a full-time basis with the option of switching to the hybrid model; or learn from home on a full-time basis until the end of the school year. We added the third option (learn from home on a full-time basis until the end of the school year) so that we know how many individuals to account for in our plans for bringing more students on campus.
3. We are asking families that have multiple students enrolled in our schools to please fill out a separate form for each one. Doing this will give you the chance to provide feedback on each child's learning experience during first semester. It will also allow you to choose the instructional model (i.e., hybrid or remote) that best meets your child's individual needs.
4. The families of students from the Transition Center do not need to fill out the form. Our staff will contact you directly to discuss plans for second semester.
We are asking that all forms be submitted by Dec. 7 at 4 p.m. Please note that if we do not receive a form for a student, we will assume that student will begin second semester in their current instructional model (i.e., hybrid or remote).
