HCCI Celebrated at the 2024 Chicago Health Executives Forum

HCCI is excited to announce that we recently received two awards from the Chicago Health Executives Forum (CHEF).

 

The CHEF Community Leadership Award, according to the CHEF website, “recognizes the accomplishments of a healthcare leader or organization in the Chicago area who has paved the way for excellence in healthcare service delivery.” HCCI’s very own Dr. Paul Chiang will be presented the award in recognition of his work as Senior Medical and Practice Advisor at HCCI and Medical Director for Northwestern Medicine HomeCare Physicians.


Dr. Chiang has made nearly 38,000 house calls to home-limited, medically complex patients throughout his 23-year career. His commitment to his patients, their families, and caregivers is evident through his personalized approach to care. For Dr. Chiang, it is about taking the time to build relationships based on mutual trust, respect, and understanding.

The CHEF Healthcare Team Innovation Award went to the exceptional team behind the Rush@Home house call program. According to the CHEF website, this award “recognizes the accomplishments of a healthcare team, as identified by the team’s leader, which has inspired leadership, change and advancement in the Chicago area healthcare community.” The HCCI team, as a co-recipient of the award, worked extensively with Rush University Medical Center to develop the Rush@Home program.


This dedicated and collaborative team has consistently demonstrated a remarkable commitment to improving patient care through innovative approaches and solutions by delivering home-based primary care to those most in need. Currently, the program serves 400 individuals on Chicago’s West Side, resulting in over 9,000 house calls. The team’s commitment to an underserved population and passion for embracing forward-thinking strategies makes them truly deserving of this prestigious recognition.

 

HCCI is honored to be among the accomplished awardees. CHEF's Annual Meeting and presentation of awards will take place on February 21 in Chicago.
